The following information will assist you in filing a claim. Please remember to sign your claim and send your paperwork to the St. Paul Pension Management Center.

Improved Pension 

  • Reinstate my pension benefits
  • Complete the appropriate Eligibility Verification Report (EVR)
  • Submit the Eligibility Verification Report (EVR)

    • Please remember that every block on the EVR needs to have a response even if the answer is zero or none

    • If you are required to submit the annual Eligibility Verification Report and you need an additional EVR form, please select the appropriate form from the following list:

  • Report an income change

    • Complete the Eligibility Verification Report (EVR) or provide the necessary information on the Statement in Support of Claim, VA Form 21-4138

    • Be sure to list the date the income changed, the amount of the income, and the source of the income

  • Report paid medical expenses

    • Complete the Medical Expense Report, VA Form 21-8416

    • Be sure to list the purpose, amount paid by you, date paid, name of provider, and for whom paid for each medical expense claimed

    • Please note that a claim for medical expenses must be received by VA no later than December 31 of the following year in which the medical expenses were paid
      • For example, a claim for medical expenses paid in 2005 must be received by VA no later than December 31, 2006, to be considered timely

  • Add my spouse as a dependent

    • Complete the Declaration of Status of Dependents, VA Form 21-686c

    • Complete the Eligibility Verification Report (EVR)

    • Please also submit a copy of your marriage certificate

  • Add my child as a dependent

  • Please also submit a copy of the child’s birth certificate

    • If you have medical expenses to report, also complete the Medical Expense Report, VA Form 21-8416

  • Report a dependency change

    • Complete the appropriate Eligibility Verification Report (EVR)

    • On the Statement in Support of Claim, VA Form 21-4138, tell us the date of the dependency change and the reason.
